How to Apply for Makati's Yellow Card

The Makati Health Plus (or the "yellow card" as it is more popularly known) is the Makati residents' health aid from the local government. The card allows the bearer free/discounted access to medical aid in the local hospital, Ospital ng Makati (OsMak) provided that the hospital bill does not go over Php 5,000.00. In case that it does, a Php 200.00 donation is encouraged (as stated at the back of the card). The city has two branches of the local hospital, one in Malugay and the other in Pembo.

When I was younger though, the LGU partnered with Makati Medical Center for this program. I remember my mother taking us to Makati Medical to get our check-ups. I, for one, was often taken to the dermatologist since I was not blessed with good skin ever! I usually had skin allergies especially during summer season. I thought we were pretty much well-off since Makati Med has always been our health-related go-to but it turns out we were not, haha! My mother used her yellow card with regards to our medical check-ups. But then again, the card itself does not mean you won't have to pay for anything. Nothing in this world is exactly free, you know? 


Fast forward, the program continued with the local hospital, Ospital ng Makati (OsMAk) as opposed to Makati Med.


There are certain classifications according to the residents' standard of living. If you can't afford medical services then you're very much qualified to avail of the program, however, you need to have all the requirements that support the fact that you are an indigent.

Here are the three categories, check out in which category you fall under and read on to know what you need to bring when you apply for the MHP card. (These requirements are for first time applicants only).

A. Family Card-  This covers the married Makati residents, married non-Makati residents employed by the city government, single parents and his/her declared/qualified dependents.

Requirements:
  1. Original and photocopy of voter's I.D or COMELEC certification (if you still have don't have your voter's ID)
  2. PhilHealth ID/ MDR
  3. Barangay Clearance (original and photocopy)
  4. Cedula/Community Tax (original and photocopy)
  5. Birth Certificate of children ages 20 and below (original and photocopy)
  6. If married, original and photocopy of Marriage Certificate; if not original and photocopy of Affidavit of Live-in Status
  7. Original and photocopy of recent Income Tax Return (ITR)
  8.  If working, original Certificate of Employment; if not, Affidavit of Economic Status
  9. Two (2) pieces of 1x1 recent picture with white background
  10. Family Picture
  11. Brown Envelope

B. Solo Card-  This type is for those of 21 years old and single, for the single non-Makati resident employed in the city government and single house-helpers.

Requirements:
  1. Original and photocopy of voter's I.D or COMELEC certification (if you still have don't have your voter's ID)
  2. PhilHealth ID/ MDR
  3. Barangay Clearance (original and photocopy)
  4. Cedula/Community Tax (original and photocopy)
  5. Original and photocopy of recent Income Tax Return (ITR)
  6.  If working, original Certificate of Employment; if not, Affidavit of Economic Status
  7. Two (2) pieces of 1x1 recent picture with white background
  8. Brown Envelope

C. For Makati City Government Employees- Refer to letter A if you are a married non-Makati resident and B if you are a single non-Makati resident.

Note: The most important requirement you need to secure is your Certificate of Employment from the Head Office or the concerned department in the city government you are working at.

When you secure all the requirements, head on to the Makati Health Program Office located at the 4th floor of the new building of Makati City Hall.

Upon passing your documents, the officer-in-charge will do a screening and advise you on what you need to do.

A home visit will then be conducted, usually a few days after you applied for the yellow card. So make sure you're around, if not, a notice of visitation will be given to whomever was home. You're application will then be on hold.


Take note that they only do up to three (3) home visits, if you miss all these home visitations, you're application will be disapproved. But if you are interviewed and satisfactorily met the requirements, you will be given a Notice of Claim. You need to bring this with you and pay for the lamination fee when you claim your yellow card.

    1. Hi to you too! But what exactly do you mean by a "just a resident in Makati" though? Are you just renting and registered elsewhere? Regardless of the voter's ID, you can just secure a COMELEC/ Voter's Certificate provided that you are a registered voter in Makati. Also, I think you should swing by the barangay hall (barangay certuficate is also a requirement) for more info on this. Thank you.

    1. Ooops, super late response. Sorry. I'm not really sure if there's a stipulated period for when you're allowed to apply for the yellow card when you're just a newly registered voter.

      I suggest you ask someone from your barangay hall or ppse your queries to the municipal personnel tasked with this for more accurate information. ��
